• Field of study
• Program start date and expected date of graduation (month and year)

Medical residents during the online registration process will need to provide:

• Name of the program in which you are enrolled
• Program director’s name and contact information
• Length of the program (in years)
• Program start date and expected completion date (month and year)

Post-doctoral fellows with an MD or PhD (in a recognized hematology or oncology training program during the online registration process) will need to provide:

• Name of the program in which you are enrolled
• Program director’s name and contact information
• Length of the program (in years)
• Program start date (month and year)
• Expected program completion date (month and year)
• Signed verification letter on official letterhead from program director or school counselor confirming the individual is a trainee or post-doctoral fellow
